چکیده

A 38-year-old woman using hormonal contraception presented with right-sided abdominal and arm clonic seizures, right hemiparesis, hypesthesia, and nausea. She denied headache. D-dimers were 350 ng/mL (laboratory reference #500 ng/mL). CT showed a small left parietal hemorrhagic infarct with adjacent hyperdense cortical vein (figure 1). MRI 15 hours after onset showed magnetic susceptibility effect on susceptibility-weighted imaging (SWI, figure 2). Isolated cortical vein thrombosis is present in 5% of patients with cerebral venous thrombosis (CVT). Negative D-dimer and absence of headache does not exclude CVT and SWI is useful for confirmation of the diagnosis. Figure 1 Baseline noncontrast CT scan

Susceptibility-weighted imaging in the diagnosis of isolated cortical vein thrombosis.

globin serves as an intrinsic contrast agent for cerebral veins which contain deoxygenated blood. Other extravascular blood degradation products with paramagnetic properties such as methemoglobin, hemosiderin and ferritin also create a contrast in SWI. In this sequence in order to increase the visualization of the smaller veins, a phase mask obtained from the magnetic resonance (MR) phase image...

Cortical Vein Thrombosis as a Mimic for Isolated Cortical Subarachnoid Hemorrhage and Transient Ischemic Attack

Isolated cortical subarachnoid hemorrhage is rare and poorly understood. Differential diagnoses and proposed pathophysiology vary widely and the diagnostic work-up for these patients who present with transient ischemic attack-like episodes and characteristic imaging findings is still unclear. We report a case of isolated subarachnoid hemorrhage and transient neurologic deficits due to isolated ...

Isolated cortical vein thrombosis - the cord sign.

Isolated cortical vein thrombosis is an uncommon condition and often difficult to diagnose, both clinically and radiologically. We report a case of a 38 years old man who presented with headache of new onset and clinical examination was unremarkable. The unenhanced brain CT did not reveal any abnormality. In view of unrelenting headache and partial seizures, we performed magnetic resonance imag...

Acute Dural Sinus Thrombosis — Computed Tomography and Magnetic Resonance Imaging Features

Dural sinus thrombosis is a diagnosis that is not commonly thought of in the first instance by radiologists and clinicians, especially when the clinical presentation is not specific. We describe the clinical presentation of three patients with acute thrombosis of the superior sagittal sinus, the left transverse sinus, and the right cavernous sinus, respectively. Compared with computed tomograph...
